What is the treatment for pregnant women with acute skinonephritis (infection of the chimney)? Hospital admission is guaranteed for any pregnant patient with skinonephritis. Treatment of choice during pregnancy includes the use of beta-lactam antibiotics. Intravenous antibiotics should be given until the patient is shaved for 24 hours and improves symptomatically. Therapy should be given for 24-48 hours or until severe symptoms improve. Once patients are feverish for at least 48 hours, they can be switched to oral antibiotics and discharged to complete 10-14 days of treatment. It is recommended to avoid fluorochinolles in pregnant patients. Aminoglycosides should also be avoided due to the potential risk of ototoxicity after prolonged fetal exposure.
